  • Download the “Individual Activities of Daily Living (ADL) Disability Claim Form” from our claims centre.
  • Download the “Assessor for Activities of Daily Living (ADL)” from our claims centre.
  • Make an appointment with a physician (please refer to our list of Assessor for Activities of Daily Living (ADL)) for the disability assessment and completion of the Doctor’s Statement. Please bring along hospital medical records and Inpatient Discharge Summary for the assessment. Do note that assessment fee and medical report fee are to be borne by policyholder.
  • Submit the completed claim form together with all supporting document to us, or through your agent/broker
  • Once we have received all the required documents, we will process your claim and inform you of the outcome as soon as possible.
  • We will inform you about the claim outcome after evaluation has concluded.

Documents required for submission

  1. Claimant’s Statement
  2. Doctor’s Statement (Assessment fee and medical report fee are to be borne by policyholder)
  3. Heart Attack (Hospital Inpatient Discharge Summary, ECG reading and Cardiac Enzymes Assays)
  4. Stroke (Hospital Inpatient Discharge Summary, CT Scan and MRI Scan results)
  5. Declaration of Beneficial Ownership (for Trust / Keyman Policies)
  6. Consent Form For Medical Report
  7. Available hospital reports, laboratory and test results, diagnostic scan reports
  8. Copy of police report (if disability is due to an accident)
  9. Certified copy of NRIC of claimant by servicing adviser
  10. Reimbursement benefit (Original bills/receipts and doctor prescription and certification)
