Follow these simple steps to expedite filing of a Death claim

When the Insured is dead and the life policy is in force, the Policyholder or the beneficiary or their representative shall promptly notify and submit the evidences of death to the Company by writing in the Company’s form which declare about the death.

  • Download the relevant “Life Insurance” Claim Form from our claims center.
  • Fill in the form and submit it and the required documents via your HR department, your service agent/broker or by registered mail to: Policy Owner Service department Tokio Marine Life Insurance (Thailand) PCL. Empire Tower 26 fl., 1 South Sathorn Road, Yannawa, Sathorn, Bangkok 10120
  • Once we have received all the required documents, we will process your claim and inform you of the outcome within 15 working days.
  • Once all the forms are completed, we will mail your payment by cheque.

Documents required for processing your claim

  1. Your Statement form(s) (Company Form 1) completed and signed by each individual beneficiary.
  2. Physician's Statement (Company Form 2) completed by an attended physician who treats the Insured before his/her death.
  3. Group Insurance Death Claim Report (Company Form 3) completed and signed by your HR personnel.
  4. A photocopy of Death Certificate signed to verify the copies by beneficiary.
  5. A photocopy of Residence Registration Certificate in which such a death is reported and a photocopy of identification card of the Insured. Both of which are signed to verify the copies by a beneficiary.
  6. A photocopy of Residence Registration Certificate of each beneficiary(ies) and photocopy(ies) of identification card(s) of the insured. All documents are signed to verify the copies by each beneficiary.
  7. A photocopy of Birth Certificate of the Insured in case that he/she is a juvenile. The photocopy is signed to verify by an authorized person.
  8. Employment Application, Working Time Card, Monthly Salary Slip of the last 3 months in which the Insured is active at work.

Additional documents required for unnatural death

E.g., by accident, suicide, being attacked by others or animals, or uknown causes.

  1. A photocopy of Police Record on the concerned accident. The document is signed to verify the copy by a Police officer on duty.
  2. A photocopy of the Autopsy Report.
  3. Copy of death certificate from the hospital or clinic.
