  • Completed Claim Form
  • Golf Club’s Incident Report to certify the incident
  • Damage Golf club(s) photos
  • Repair or Replacement Bill
  • Completed Claim Form
  • Golf Club’s Incident Report to certify the event
  • Third party’s claim notice including with supporting evidences
  • Completed Claim Form
  • Golf Club’s Hole-in-One Certificate
  • Golf Score Card to indicate the Hole-in-One
  • Other (if required)
  • Completed Claim Form
  • Golf Club’s Incident Report to certify the Accident
  • Original Medical Bills/Receipts
  • Original Doctor Medical Report

What to do after an accident

  1. Notify claim to Insurer as soon as possible
  2. Notify Golf Club the claim event to provide additional evidences
  3. In case of theft or Golf’s missing, the Insured must notify the police and take one copy for Insurer evidence
  4. In case Third party’s property damage and bodily injury, the Insured shall not accept all liability or any admission in connection with the accident without Insurer’s consent
  5. Maintain all of damage properties for Insurer’s inspection
  6. Provide all of claim evidences as requested by Insurer
