Death Claim Process

Follow these simple steps to expedite filing of a Death claim

As a TMLTH customer, you are entitled to privileged service at any of our network hospitals. To enjoy this privilege, just show your TMLTH Care Card with your valid identity card. Whether you are admitted or seeking an out-patient consultancy, we will pay the medical expenses incurred defined in your plan directly to the hospital.

If you visit any other hospital, you will have to pay for the medical expenses in advance, and make a claim for reimbursement. So avoid the hassle, and go straight to our network hospitals, every time when need to visit a doctor.

How to get a Claim Reimbursement

  • Report your claim

  • Submit form and required documents

  • Evaluate claim

  • Close claim


  • Report your claim to your respective agent or customer service in writing as soon as possible.
  • Fill in the form and submit it and the required documents via your service agent or by registered mail to: Individual Claim Department Tokio Marine Life Insurance (Thailand) PCL. Empire Tower 26 fl., 1 South Sathorn Road, Yannawa, Sathorn, Bangkok 10120
  • Once we have received all the required documents, we will process your claim and inform you of the outcome as soon as possible.
  • Once all the forms are completed, we will mail your payment by cheque.

Documents required for processing a claim

  1. Death Claim Form
  2. Physician's Report
  3. Death Investigation Report prepared by Agency Leader/Agents 
  4. Original Death Certificate and a copy
  5. The original and a copy* of the House Registration Certificate with notification of death of the Insured
  6. The original and a copy* of the Insured’s Identity Card
  7. The original and a copy* of the House Registration Certificates and Identity Cards of all beneficiaries
  8. Insurance policy 

Additional documents required for unnatural death
e.g. by accident, suicide, being attacked by others or animals, or unknown causes. 

  1. Copy of Police Report issued by Royal Thai Police
  2. Copy of Autopsy Report
  3. Investigation Report on the body (in the event of an autopsy)
Note: All copies of documents must be certified by the beneficiary or the beneficiary’s parents if the beneficiary is a minor.

Payment Method

Payment will be via:

  • Direct credit to your bank account (please submit a copy of your bank book) or
  • A cheque for your account only will be mailed to your address, as in our records.

The claim will be settled within 3 working days from the time of submission of completed documents.  In case of any investigation required, the claim will be settled within 30 – 90 days, depending on the investigation period.
