As a TMLTH customer, you are entitled to privileged service at any of our network hospitals. To enjoy this privilege, just show your TMLTH Care Card with your valid identity card. Whether you are admitted or seeking an out-patient consultancy, we will pay the medical expenses incurred defined in your plan directly to the hospital.
If you visit any other hospital, you will have to pay for the medical expenses in advance, and make a claim for reimbursement. So avoid the hassle, and go straight to our network hospitals, every time when need to visit a doctor.
Download the relevant “Life Insurance” Claim Form from our claims center.
Fill in the form and submit it and the required documents via your service agent or by registered mail to:

Once we have received all the required documents, we will process your claim and inform you of the outcome as soon as possible.
We will inform you of your payment via sms.
Documents required for processing a claim
Completed Claim Form
Original receipts of expenses and summary of medical expenses (receipts must be “certified true copy” by a hospital officer)
Any other medical documents e.g. medical examination (if any)
Payment will be via:
Direct credit to your bank account (please submit a copy of your bank book) or
A cheque for your account only will be mailed to your address, as in our records.
The claim will be settled within 3 working days from the time of submission of completed documents. In case of any investigation required, the claim will be settled within 30 – 90 days, depending on the investigation period.
To avoid unnecessary delays, make sure your claim form is fully filled in, and all other required documents are complete.
Ensure your signature on the claim form tallies with the signature in our records.
The Medical Report is to be completed at your own expense.
