Submit the completed claim form together with all supporting document to us, or through your agent/broker
Once we have received all the required documents, we will process your claim and inform you of the outcome as soon as possible.
We will inform you about the claim outcome after evaluation has concluded.
Documents required for submission
Consent’s Form for Medical Report
Certified true copy of death certificate (refer to Note A below)
Proof of relationship between claimant and deceased (refer to Note B below)
Copy of NRIC(s) of claimants
Additional documents required for accidental / unnatural death or for death occurred overseas:
Doctor’s Statement (required for overseas death)
Newspaper clipping (if any) and police report (if death was a result of accident / unnatural death)
Burial cremation documentation (required for overseas death)
Letter from Immigration and Checkpoint Authority (ICA) for overseas death (refer to Note C below).
All documents that are not issued in Singapore must be authenticated by the Singapore Embassy in the country of death, Singapore Consulate or Notary Public. (Refer to Note D below)
(A) For death which occurred in Singapore, original death certificate can be certified true copy by our Customer Service Officers or Claim Staff of the company or your lawyer. For death which occurred overseas, death certificate can be certified true copy by our Customer Service Officers or Claim Staff of the company or your lawyer or any Notary Public.
(B) All proof of relationship can be certified true copy by our Customer Service Officers or Claim Staff of the company or your lawyer
(C) For Singaporeans and Permanent Residents (PR) who died overseas, ICA would issue a letter confirming receipt of deceased’s Singapore NRIC, Passport and overseas Death Certificate, and invalidation of deceased’s Singapore NRIC / Passport. Please submit a copy of this letter to us.
(D) All documents which are in foreign language must be officially translated to English (translated by official Authority / Notary Public / Embassy) before submitting to us
