Death Claims Process

Follow these simple steps to expedite filing of a death claim

  • Please click on Login > Life Insurance e-Services.
  • Please submit via the "Submit Death Claim" menu, if you are the next-of-kin.
  • Please login to the customer portal if you are the policy holder, to submit death claim for Life Assured.
  • Follow the 4 simple steps to submit death claim.
  • Upon successful e-Claims submission, Next Of Kin will receive an e-Claims Receipt Acknowledgement in the form of screen message, SMS and email.

Documentation Required

Mandatory Documents 

  1. Death Claim Form
  2. Death Certificate
  3. Deceased's NRIC
  4. Claimant's NRIC
  5. Claimant's proof of relationship with decease (marriage certificate/birth certificate)

Supporting Documents (whichever applicable)

  1. Doctor's Statement (required if policy is in-force for less than 5 continous years)
  2. Police Investigation Report (death due to accident/suicide/murder)
  3. Toxicology Report (death due to accident/suicide/murder)
  4. Post Mortem Report (death due to accident/suicide/murder)
  5. Letter of Administration or Grant of Probate (required if no nomination was made under the policy)

Documents Required If Death occured in oversea

  1. Foreign Death Certificate / Letter certified by the Malaysian Embassy-overseas
  2. Certified True Copy of letter of confirmation of death by JPN
  3. Import / Export  Permit Certificate
  4. Coffin permit / Tranship the remains of a deceased person issued by the Jabatan Kesihatan Malaysia 

Important Notes Prior to Submission

  1. Ensure completion of claim forms and completeness of all document to avoid unnecessary delay.
  2. The medical report is to be completed at the claimant’s own expense.
  3. The checklist provides a reference of the point-of-submission requirements only. The company reserves the right to request for other relevant documents and information.
